Chronic Disease Management Plan
For any patient with a chronic condition lasting 6+ months that is being managed by their GP through a GP Management Plan (GPMP) or Team Care Arrangement (TCA).

How To Get A GP Referral For Medicare Rebates
The M10 & GCCMP pathway has three defined stages, each with specific requirements. Here's exactly how it works from referral to treatment.

Get a Referral from Your Doctor

You explain your child’s speech or communication concerns.

The doctor decides which fits best:

M10 – for under‑25s with certain diagnoses.
-
GPCCMP – for ongoing (chronic) conditions, any age.

The doctor writes a plan
M10 (for under‑25s):

GP/paediatrician sends a referral for assessment to the speech pathologist.
-
After assessment, they may write an M10 Treatment & Management Plan with diagnoses and therapy goals.
GPCCMP (for chronic conditions):

GP writes a Chronic Condition Management Plan.
-
It lists your diagnoses, goals, and which health professionals are involved.
In both cases, you get a copy of the plan.

Referrals to Speech Therapy
With the plan in place, the doctor writes referrals to speech pathology.
M10:

Up to 8 assessment sessions (lifetime, shared across allied health).
-
Up to 20 treatment sessions (usually two lots of 10).
GPCCMP:

Up to 5 Medicare‑rebated allied health visits per year, shared across speech/OT/physio, etc.
You then book with your speech pathologist and use these referrals to claim rebates.

Go To Sessions & Get Updates

You attend speech sessions (in‑person or telehealth).

The therapist works on the goals in the plan and gives home practice ideas.

At the end of a block, the therapist writes a progress report back to your GP/paediatrician.

Review & Decide What’s Next

The GP reviews how things are going.

Together you decide whether to:

use more Medicare sessions

renew or change referrals

add other funding (NDIS, private health, self‑funded).
